You can get them from the Peoria Health Department or the Courthouse
SOCIAL SECURITY CARD
PHYSICAL AND UP-TO-DATE- IMMUNIZATIONS Physical must be dated after August 25, 2008
DENTAL must be dated after November 15, 2008
VISION EXAM needs to be completed by October 15th

Attendance Club
We have started a new attendance club! If your child
attends school everyday, they will be come a member of the attendance
club for the month and receive a special treat. Also, each club
member's parent will be entered in a drawing for a $25 gift card!
